SUMMARY OF SERVICES
(Egg Donation)
For many couples, using an Egg Donor significantly increases the chance of achieving a pregnancy. Barring any other fertility issues, clinics typically quote pregnancy rates of 80%/up when Donor eggs are used. Happily, we have found this to be the case as well and have had the privilege of assisting countless couples to realize their dream of parenthood.

Upon receipt of Recipients' Questionnaire, which contains their preferences and requirements with respect to an Egg Donor, the Profiles and photos of Donors that meet their criteria will be provided to Recipients for review. The information will include the Donor's answers to a broad range of questions designed to provide information relative to her physical, emotional, and reproductive history. Recipients are provided with as many profiles of candidates as they wish to see. In the unlikely event that none of Baby Miracles' available Donors meet their needs, specific advertising will be placed, at Recipients' expense, to locate one who does. Once Recipients choose a Donor, the Donor will be provided with the Profile portion of Recipients' Questionnaire (which contains no confidential or identifying information). If the Donor and/or Recipients wish to speak by phone or meet in person, the agency's assigned Coordinator will make the necessary arrangements. Please note that no contact or identifying information will be shared between the Egg Donor and Recipients.
The vast majority of egg donation arrangements are anonymous in nature and the parties never meet or speak with one another. However, in cases where the Donor and the Recipient couple wish to either meet or speak by phone, this will be arranged. The Baby Miracles Case Coordinator will initiate and participate in the meeting or conference call.
Once the Recipients and a Donor agree to work together, a procedures letter setting forth the various steps from that point until retrieval will be drafted and sent to each party.
Baby Miracles holds a health and life insurance policy for with AIG Insurance through Brown & Brown of Texas to cover medical complications the Donor may experience as a result of the egg donation cycle. Premiums are paid on a per-cycle basis, following execution of the Egg Donation Contract. The Case Coordinator will file all paperwork needed to establish the cycle policy for the donation cycle if the Recipients' clinic does not include this coverage in the cycle fee. If Baby Miracles' AIG policy is used, the Case Coordinator will file the appropriate paperwork to establish the cycle policy as well as handle insurance claims to ensure they are paid.

Contract Between Recipients and Egg Donor
A Contract between Recipients and their Donor must be signed prior to the Donor starting medications. Recipients have the option of having an attorney draft the Egg Donation Contract and paying their attorney fees and those of their Donor or utilizing Baby Miracles' standard contract at a significant discount. If an attorney drafts the Contract, the Donor is required to be represented by an attorney also. If Recipients elect to utilize Baby Miracles' standard contract, the Donor may elect to waive attorney representation.
The Case Coordinator will draft the Egg Donation Contract according to terms agreed upon by both parties and forward to Recipients for review. Once approved, the Contract will be forwarded either to the Donor directly (if she waives attorney representation) or to the Donor's attorney, as the case may be. The Case Coordinator will review the contract with the parties and make requested/agreed upon changes.

Fertility/Monitoring Clinic(s)
If the Recipients are not already working with a fertility clinic, we will either recommend one we have worked with or locate a clinic in close proximity to where the parties live. If the Egg Donor lives within a reasonable driving distance to the Recipients’ clinic, she will be seen at the same clinic. If not, we will locate a monitoring clinic for the Egg Donor's screening and cycle coordination. The Case Coordinator will provide the clinic(s) with the contact information for the parties.
Prior to the Recipients and their Egg Donor entering into a Contract, the Donor will undergo medical screening to determine her suitability as an Egg Donor. This testing will include a Federally-mandated communicable disease panel.
Prior to the Recipients and the Egg Donor entering into a Contract, the Donor will undergo a psychological evaluation to ensure her suitability as an Egg Donor. The fertility clinic may also require the Donor's husband/partner and/or the Recipients to undergo psychological screening as well. We will recommend or locate a psychologist, assist in appointment scheduling, communicate with the psychologist with respect to his or her findings, obtain the appropriate report and letter of clearance, and provide the clearance letter to the fertility clinic.
Medical Records and Donation History
The Case Coordinator will request and obtain medical records reports relating to the Donor's prior egg donation(s) and forward this information to the clinic for review before the cycle calendar is set.
The Case Coordinator will arrange for a trust account to be opened and provide Recipients with an itemization of the expenses to be paid out of the account. The Case Coordinator will timely requisition payments of expenses as they become due throughout the Egg Donation arrangement. A detailed report will be provided to Recipients upon completion of donation. The trust account must remain open for a period of no less than 90 days from date of retrieval to allow a sufficient amount of time to receive all invoices and ensure all donation-related expenses have been paid.
Cycle Coordination; Communication
The Case Coordinator will communicate with the fertility clinic(s) as to the cycle coordination and medication protocols of the parties, monitor the cycle progress, and facilitate communications between the Recipients, the Egg Donor, and the clinic. The Cycle Coordinator will keep the Recipients apprised each step of the way.
Cycle Medications and hCG Conference Call With Donor
Although some clinics will provide fertility-related medications, most do not. There are a variety of pharmacies from which these medications can be obtained, including discount pharmacies in the U.S. and abroad. We can recommend pharmacies and assist in ordering and arranging payment of the prescribed medications.
The Case Coordinator will schedule a conference call with the Donor just prior to her hCG injection (to "trigger" ovulation) to ensure she or the person administering the injection understands the proper way to mix the medication (if necessary) and take this injection.
If the Recipients' clinic is not local to their chosen Donor, the Case Coordinator will make make all travel arrangements for the Donor, including transportation via airplane or train, hotel accommodations, and rental car. Additionally, the Donor will be sent a check for expenses prior to each trip.
Moral and Emotional Support
The Case Coordinator will provide ongoing moral and emotional support and assistance to all parties throughout the donation arrangement.
Upon conclusion of the egg donation arrangement, the Case Coordinator will provide Recipients with a detailed report of trust account disbursements, along with a refund of the balance of unused funds.
